Sample questions and guidelines from mystery diner evaluation forms:
Please describe your telephone conversation (when calling to make the reservation).
At the restaurant, were you greeted with a smile and good eye contact?
Was the manager visible, circulating and professional?
Did the bus person acknowledge you either by smile or eye contact or by saying hello?
Did you receive your appetizer, soup or salad within seven minutes of ordering?
Did you receive your entrees within 15 minutes of ordering? Or within 10 minutes of finishing soup, salad or appetizer?
Did you feel the entree portioning was adequate?
When commenting on the appetizers, entrees or any other items, please try to describe the ingredients and flavor as though the person reading it has never tried the items before.
